You need to be living within our catchment area to register, please check that your postcode is within our practice boundary. If you live slightly out of area you may be in the catchment area for our sister site: Berrylands Surgery (GP practice in Surbiton).

Please note once we have received your registration we will email you requesting proof of ID in the form of a passport or driving licence, proof of address (dated within the last 3 months) and proof of parental responsibility if registering someone under the age of 16years.
Please note to register a child a person with parental responsibility must first be registered at the surgery.
