There are many different ways you can access your medical records. We recommend the NHS app. If you have the NHS App or another application that provide a similar service (such as Patient Access) then you have online access.

Canbury Medical Centre has disabled our patients from being able to book appointments through the NHS app. We use a triage system to ensure all appointments are booked in with the correct clinician and in the most appropriate time frame.

In order to access the NHS App patients need to have an email address and mobile number linked to their medical record.
We offer online access to most of our patients. The only exception is children aged 11 - 16.
If you are between the ages of 0 and 10, your parents can be set to view your medical record online. This type of access is called ‘Proxy access for a child’.
Once a Child is 11, proxy access is declined for parents as 11 year olds are entitled to medical confidentiality. Once a child reaches 16, they can be set up for their own online access. You can read more about this topic on our practice policy page.
If you are a parent or guardian and would like to be set up with proxy access for a child. Please download this form and return it to us with a form of ID.
Adults can also hold proxy access for other adults. This type of access is called ‘Proxy access for an Adult’. There are many reasons why this may be necessary for some patients.
For example, many of our older patients have allowed their adult children to have proxy access to allow them to place prescription requests on their behalf.
